Withdrawal request

Before you submit this withdrawal request, please ensure your child has completed any makeups. They must be actively enrolled to do so. Should you want to transfer to a different class, please contact us at info@losgatoselite.com.

Please submit this form for each child you wish to withdraw from class before the 25th of the month to withdraw for the upcoming month. Withdrawals made after the first of the month will result in credit only. Once submitted, your child will be removed from the requested classes.
